Physicals will be administered on July 15, 2026, and July 22, 2026, between 1:00pm and 4:00pm, at the high school nurses’ office. There will be 10-12 students seen per hour. The first 10-12 who sign up during that time slot will be guaranteed the time they choose. The student-athletes will be seen by appointment only. You will be notified of your time if you are not in the first 10-12 to register for that specific time slot. Once the time slots hit capacity, they will be removed from the Google Form. These time slots should be reserved for students without a steady primary doctor. Please download the physical forms linked below and be sure to complete the appropriate forms for the doctor. The first three pages must be completed before coming in for the physical. The doctor will complete the final two pages. If you need to cancel, please do so at least 48 hours before so that the spot can be used for someone else.
